To shorten the length of their exam, first time patients should complete the Patient Information Form prior to their visit.

Adult or Child Patients
The form can be completed electronically or manually. When the form is complete, it can be sent to us via email or fax or bring a printed copy to the exam. If emailing or faxing the form, please be sure to select the correct "send" button at the end of the form or the fax number for the office where the exam is scheduled.
